Farmhouse Grain Request

FORM INSTRUCTIONS – Please Read

To obtain grain, malt extract, or yeast please fill out form below and make sure all required fields are filled before submitting. Other instructions or limitations are as follows:

1. Supply a grain bill for one 5 or 10 gallon recipe.

2. Bring some of your finished product to a club event to share. A Growler or six pack minimum.

3. Limit on grains is four types per recipe. You can add more on your own.
4. There is a limit of four 5-gallon recipes or two 10-gallon recipes per member per year.
5. There is a limit of 15 lbs total grain/malt for a 5-gallon batch and a 30 lb limit for a 10-gallon batch per request.
6. For Extract and Partial Mash Brewers there is limit based on malt cost equivalency that adheres to the limits above. Details can be found in the extract section of this form.
7. Please give at least 3 working days notice.
8. Farmhouse Brewing Supply is closed on Sundays and Mondays.
9. One yeast package per 5-gallon recipe and two for a 10-gallon recipe.
10. Outside of the grain and yeast as explained above you’re on your own for any other items or ingredients. However you can add these other items in the field provided towards the bottom of this form. These items will need to be paid for by the member by contacting Farmhouse Brewing Supply directly.
11. If your grain request is approved you will recieve an email confirmation. Pickup date/time will be confirmed by Farmhouse Brewing Supply. Members who fail to pickup an approved requested order from the selected location may be removed from this program.

If you need dry or liquid malt extract please use the drop down fields below. Each selection has a package weight displayed. Please remember you are allowed a total malt cost equivalency of 15 pounds for 5 gallon batches and a total malt cost equivalency of 30 pounds for 10 gallon batches.

For the sake of calculating malt cost equivalency:

• 1 lb of DME is equivalent to 3 lbs grain
• 1 lb of LME is equivalent to 1 ⅔ lbs grain
